Forget Me Not Care offers Quality, Specialised Dementia Care within a persons own home promoting independence, dignity and respect at all times. We hand pick professional, highly trained staff who strive to develop a relationship with service users and their families to enable us to deliver a package of care that is tailored to each individual.

We offer a range of assistance of care and support to include the ongoing management of an individuals dementia, whilst supporting them to maintain thier independance and where possible remain in thier own homes. Some aspects of this support may include;

  • Personal care -this may be assistance with getting up in the morning or with going to bed in the evening,Bathing/showering and Dressing.
  • Prompting taking medication from a pre packed by a pharmacy blister/dosage box
  • Hospital Visits accompanying to and from hospital of G.Ps appointments.
  • Domestic Help this could include, Laundry, ironing and Shopping support.
  • Social Trips Out this enable a service user to feel less isolated at home.
  • Meal Preparation this could include monitoring dietary and fluid intake.
  • Sleep-in-service when a service user needs support during the night as well.

Respite Care from time to time Primary Carers need a break to relax, go on holiday or just take a few hours off. We provide long or short term respite assistance at a persons own home. We understand that to take time out from a loved one can be a difficult decision for Primary Carers and can be a very apprehensive time so we aim to make it as stress free as possible for both family and service user.

We provide Care every day of the week all year round. Care packages can be as little as 30 minutes or as long as 24 hours. We  can also provide a sleepin service, to a waking night, from initial point of contact we work closely with our service Users and their families/friends and other professionals to find out more about them as individuals and to work out their exact requirements a comprehensive care needs assessment will be carried out.  Our aim is to ensure that the package provided is the best it can be to support the client and meet thier needs to the highest standard we can achieve. We also have regular meetings and updates to meet with the clients, family members and any other professionals involved to assess the package and whether it still meets all needs or if changes are required. We are very aware that listening to both service user and family ,can make all the difference in the success of the care that we provideand is absolutely critical to offering a holistic service to a person with Dementia and other Mental Health related issues. It is also critical that we are completely aware of all behavioural changes that may occur in order to manage these challenges  with as little disruption to the person and thier security as possible. Our person centred approach ensure the best possible outcome for our service users in accordance with the Care Quality Commission with whom Stroud Care Service are regulated. We aim to be able to respond to a request for care with in a 24 hour period where possible.
